Warranty Claim Form

Warranty claims are submitted by the wholesale distributor on the claim form. Download it, fill it out and return it to Mortex.

Before you send a claim

Only authorized wholesale distributor claims are accepted. Allow 30–45 days for processing.
Warranty parts must be returned to the same distributor as the replacement item was purchased. Warranty claims must be submitted by the same distributor. All other submissions will be denied. Mortex will only issue warranty credits to the replacement part selling distributor.
Claims submitted over 120 days after the failure date are considered unacceptable and invalid.
Failure to provide verifiable complete serial number is cause for warranty denial. Include a photo of the serial number label from the case; a label without the serial number is not acceptable.
Product registration is required for product over one year old when installed, otherwise warranty term starts at six months from manufacture date. Register a product.
